The Cistern - Secret #3
Find the movable block to the left of the entrance. (It's outlined in the screenshot above.)
Push it once and enter the room behind. This is secret #3.
Stand underneath the balcony with Lara's back toward the slope. Step back to make sure she's as close to the base of the slope as possible. (The correct spot to stand is marked with an X in the screenshot above. The camera angle prevented me from getting a good shot with Lara actually standing there.)
Backflip onto the slope and then hold Jump and Action to spring forward off the slope and grab the edge of the balcony. Pull up and get the goodies.
