The four-player puzzle is shown below. I have not played the 3-player version yet, but I am guessing it is similar. One character would then need to stand in the middle and activate each pressure pad in turn, while another controls the grapple, and the third stands on the cable. In this version there is a grapple ring near the entrance which is not necessary to solve the puzzle. There are also 2 pressure pads on the platform in the middle of the spike pit. The pad on the right deactivates the ice vent blocking the walkway on the right side of the room. The pad on the left deactivates the ice vent on the far side of the room. The ice vent on the left side of the walkway remains on, so you'll have to work around it.
Have one character jump across the spikes onto the platform with the pressure pads. (If you have a fourth character, he or she can do this as well, but it isn't necessary.) Then another character (here it's Lara) should stand near the golden ring and grapple the character standing on the pad. With the cable extending across the deactivated ice vents, the third character (Horus) can then jump onto the grapple cable and balance on it above the spikes.
The character holding the grapple should now very carefully move around to the right, while the one in the middle (Isis) stands on the right pressure pad to deactivate the right vent, and the one standing on the cable adjusts her position in order to stay between the ice vents surrounding the pit.
As the character on the outside rounds the corner and approaches the ice vent on the far side of the room...
...the one in the middle should move off the right pad and onto the left pad to deactivate this vent. The character holding the grapple can then continue to the left...
...round the next corner, and then stop just before the ice vent on the left.
From this position, the character on the cable should be able to jump over to the walkway on the left and claim the reward: the HEAVY GRENADE LAUNCHER and a MAX HEALTH UPGRADE.
Now either return around the edge of the room or jump over to the center platform then back to the ledge near the switch. Leave the tomb the way you came in.
