Please keep in mind that I only have the PC version of these games, so I can't vouch for the accuracy of any of the console cheats. Also, I have received reports that certain cheat codes (e.g., unlimited shotgun ammo) don't work. If I have posted an incorrect code, please let me know.
Cheat codes can only be unlocked by first beating the game. (Yes, I realize this makes no sense. Sorry.) After completing the Bolivia Redux level, the Cheat Codes sub-menu is accessible by selecting Extras > Special > Cheat Codes from the main menu.
Then unlock each individual code by completing levels in Time Trial. Finally, to activate a code, press the keys indicated during game play. Note that you can enter more than one code at a time (though only one weapon code at once), and you can enter codes during cut scenes. When you enter a code successfully, a chime sounds. To de-activate the code, press the same keys again. A complete list of codes is below.
Nintendo DS & Game Boy Advance Cheat Codes: These work a bit differently. They are listed below the main codes section.
UPDATE: In the PC version, at least, it is possible to apply certain cheat codes before beating the game. In the Xbox 360 version, after beating the game once and activating the cheats, you may be able to load earlier saves and continue using the cheats. For instructions, see below. Thanks to yellowflyingcows for the original tip and Sanne for the Xbox info. The GameCube version (playable on GC or Wii) also includes a work-around for using the cheat codes on a second play-through. Again, see below. Thanks to Hillska for this.
ALTERNATIVES TO CHEATING:
Useful 100% Health Bug: Though not actually a cheat, it may be useful to note that Lara's health bar is refilled whenever the game reloads at a checkpoint. So if she's near death, try to hang on just until the next checkpoint. Then let her die, reload and she'll be back at 100%.
Savegame Files: PC players are welcome to download a savegame file with all the codes unlocked. Or, if you're hung up in a certain spot and the walkthrough just isn't helping, download a savegame file for a checkpoint just beyond where you're stuck. PC save files are at https://tombraiders.net/stella/savegame/TR7saves.html.
PlayStation and Xbox saves are available at GameFAQs.com.

PC GAME CHEAT GLITCH - APPLY CHEATS BEFORE BEATING THE GAME
NOTE: You should not need to re-enter the cheats once you've loaded the earlier save, but you will need to reload the 100% save, enable cheats and then switch saves each time you shut down and restart the game. Again, very special thanks to yellowflyingcows for sharing this discovery.
XBOX 360 CHEAT GLITCH VARIATION
You still need to beat the game and unlock the cheats normally; however, once you've activated the One-Shot Kill cheat, you should be able to quit, load a different save with the game not yet completed, and the cheat should remain active. You may need to re-enter the button combo for the cheat after completing a level and moving on to the next, but it should still work. I'm not sure if this works with other codes besides the One-Shot Kill. Let me know how it goes for you. Thanks to Sanne for this useful tip.
GAMECUBE/WII CHEAT GLITCH VARIATION
As in the other versions, the cheats are not available until you have finished the game. But if you finish a game, then load a level, input any of the cheat codes (except the ones involving specific weapons, since those don't seem to work). Then exit the level, start a new game, and those cheats should carry over so you can use them in the new game. They will last as long as you don't completely quit and exit the game. (That is, don't turn off the GC/Wii or play another title.) Thanks to Hillska for this info.
After completing either game you can purchase extras using rewards you have picked up throughout the game, two of which are cheat codes. Once they are purchased, enter the codes during gameplay.

*NOTE: The bulletproof cheat makes Lara impervious to enemy attacks and drowning (except in the Nintendo DS version, when she can still drown). She can still be injured by falling, freezing water, etc., however.
**FYI: In case you're not familiar with it, Soul Reaver is the weapon from the Legacy of Kain series developed by Crystal Dynamics and published by Eidos.
Special thanks to Christopher, Heathyr, Sonia, Turza and Treeble for providing the correct codes for the "big" game, to James for the DS codes, and to Yellowflyingcows, Sanne and Hillska for the various cheat exploits.
